Our fellowships collect a voluntary self-declaration from attendees. Forms are opened through your fellowship's own QR code or link.
This form records that your participation is voluntary. It is provided for information and record-keeping, and is not legal advice.
हे स्वरूप तुमचा सहभाग स्वेच्छिक आहे हे नोंदवते. हे माहिती आणि नोंद ठेवण्याच्या उद्देशाने दिले आहे, आणि हे कायदेशीर सल्ला नाही.
What you will confirm
1.मी माझ्या स्वतःच्या इच्छेने आणि निवडीने उपस्थित राहत आहे / सहभागी होत आहे.
I am attending / participating of my own free will and choice.
2.उपस्थित राहण्यासाठी किंवा माझा धर्म बदलण्यासाठी मला पैसा, भेटवस्तू, नोकरी, शिक्षण, विवाह, भौतिक लाभ किंवा इतर कोणतेही प्रलोभन अथवा आमिष देण्यात आलेले नाही.
No money, gift, employment, education, marriage, material benefit, or any other inducement or allurement has been offered or given to me to attend or to change my religion.
3.माझ्यावर कोणतीही जबरदस्ती, दबाव, फसवणूक, चुकीची माहिती, धमकी किंवा अवाजवी प्रभाव वापरण्यात आलेला नाही.
No force, coercion, fraud, misrepresentation, threat, or undue influence has been used on me.
4.मी कोणत्याही वेळी कोणत्याही परिणामाशिवाय निघून जाऊ शकतो/शकते किंवा सहभाग मागे घेऊ शकतो/शकते हे मला समजले आहे.
I understand I may leave or withdraw at any time without any consequence.
5.हे घोषणापत्र मी समजत असलेल्या भाषेत (मराठी / हिंदी / इंग्रजी) वाचले आणि समजून घेतले आहे.
I have read and understood this declaration in a language I understand (Marathi / Hindi / English).
6.नमूद केलेल्या अनुपालन उद्देशासाठी डिसायपल्स कम्युनिटीने ही नोंद जतन करण्यास मी संमती देतो/देते, आणि माझ्या माहितीमध्ये प्रवेश, दुरुस्ती किंवा ती मिटवण्याचे माझे अधिकार मला समजले आहेत.
I consent to Disciples Community storing this record for the stated compliance purpose, and I understand my rights to access, correct, or erase my data.

Why we keep this record: to demonstrate that participation in our gatherings is voluntary, as required by law. We collect only what is necessary. Your record is stored securely in India, is visible only to authorised Disciples Community administrators, and is never sold or shared for marketing. You may ask us to access, correct or erase your data at any time by contacting our grievance officer.
